Joining up care for people and populations
Seamless Health and Social Care at a point of Place is the subject of much debate and discussion at present. While progress has been made in many different areas, the system remains fragmented and too often fails to deliver joined-up services that meet people’s needs. The goals of different parts of the system are not always sufficiently aligned to prioritise prevention, early intervention and population health improvement to the extent that is required.
